Went in for chipped tooth.  CAme out with a temporary crown and fears for catching different diseases.  He wrote with an old pencil onto the temporary crown. 

Never wrote an itemized bill.  Still don't know what I paid $680 for???

Didn't get a mold of my teeth to replace missing crown.  When asked about it, first he says 'Let me correct that.'  Next says, 'Couldn't get it because of a cavity near the tooth.  I know that that was a lie.

Would not give me a 'copy' of the X-Rays.  Instead made me sign out the originals because he could not 'copy' them.

Did not have x-rays on the tooth that he decided needed a crown. 

Did not give me a HIPPAA document even though I signed for it.

Hygenist didn't seem to know what she was doing.

Medical equipment was broken (chairs, dental mirrors, etc).

I hope deeply that someone stops him from caring for patients before he seriously hurts someone.
